Licensed Practical Nurse

*Part-time evening short shift 5:30 p.m.–10:45 p.m. Includes every other weekend rotation.

Responsibilties:
The primary purpose of this position is to provide direct nursing care to the residents, and to be in accordance with the current federal, state and local standards, guidelines and regulations that govern the facility. The LPN Charge Nurse will administer medications, treatments and other physician ordered cares.

Requirements:
Must maintain a current Minnesota LPN licensure. The LPN position requires exceptional customer service skills and ability to work effectively with changing priorities. Must understand and demonstrate the importance of being a team player and must possess ability to maintain confidentiality.

Director of Nursing
Clarkfield Care Center

Responsibilities:
The Director of Nursing (DON) manages all functions within the Nursing Department including resident cares, nursing staff and training, managing resources and establishing standards, goals, objectives to assist residents with achieving or maintaining their highest possible level of physical, spiritual and psychosocial well-being. The DON reports to the Executive Director. The DON promotes effective working relationship with the multi-disciplinary team, residents and family, and various members of the medical community, those providing care to the resident, and professional colleagues. The DON oversees all nursing staff.

Requirements:
The Director of Nursing must be currently licensed as a Registered Nurse in the State of Minnesota. Five years of management and related nursing experience is preferred. The DON must possess strong management and organizational, communication, analytical and clinical skills. The DON must have an ability to lead and work effectively with residents and families, coworkers, and staff who collectively possess a variety of personalities and backgrounds. The DON must have a solid understanding of federal and state regulations and guidelines affecting nursing home care. He/she must have strong computer skills. The Director of Nursing is also required to have an ability to read, write and speak English, and complete and pass a state required background study form which checks for certain criminal convictions or serious/recurring maltreatment.

Clarkfield Care Center seeks a Nursing Home Administrator to join its team. Clarkfield Care Center consists of 36 nursing home beds and is a Medicare-certified home health care agency located in Clarkfield, Minnesota.

The City of Clarkfield, Heart of the Prairie, is a family-friendly community with a big heart in Minnesota’s premier farming region.

The Nursing Home Administrator is responsible for overseeing the daily operations of a care center and related business lines. The responsibilities will include staffing, financial management, supervising resident care, marketing and community outreach. Strong knowledge of federal, state and local standards for long-term care is essential in this position.

Job Functions:

I. Resident Care Management
A. Monitor and evaluate residents’ satisfaction of the quality of care and quality of life through communications with residents and/or their representatives.
B. Plan, implement and evaluate programs to ensure that high standards of care are maintained.

II. Personal Management
A. Establish a work environment to foster communication between management and all staff.
B. Recruit, train, evaluate and retain individuals to provide resident care and services.
C. Audit procedures to recruit, train, evaluate and retain individuals to provide resident care and services and communicate results to appropriate individuals.
D. Plan, implement and evaluate personnel policies.
E. Plan, implement and evaluate employee health and safety programs.
F. Develop and implement employee solution system.

III. Financial Management (ensure financial viability in order to provide services needed by our residents)
A. Develop and manage a budget for the facility to allocate fiscal resources.
B. Develop and manage financial audit and reporting systems.
C. Communicate the financial performance in a timely manner to the Board of Directors.
D. Manage accurate case-mix assessments.

IV. Environmental Management (ensure a safe, clean and attractive environment)
A. Plan, implement and evaluate a system for maintaining and improving buildings, grounds and equipment.
B. Plan, implement and evaluate a program that provides a clean, attractive and homelike environment for residents, staff and visitors.
C. Plan, implement and evaluate an overall safety program which will ensure the health, welfare and safety of residents, staff and visitors.
D. Plan, implement and evaluate fire, disaster and emergency programs to protect the safety and welfare of residents, staff and property.

V. Regulatory Management (comply with laws, regulatory codes and governing entities’ policies)
A. Plan, implement, monitor and evaluate policies and procedures to maintain compliance with laws and regulations.
B. Plan, implement, monitor and evaluate policies and procedures to maintain compliance with directives of governing entities (e.g. Board of Directors)

VI. Organizational Management (ensure integration of all aspects of the operation through leadership and direction)
A. Observe, monitor and evaluate outcomes of all programs, policies and procedures of the facility to ensure effectiveness.
B. Implement, monitor and ensure the integration of residents’ rights with all aspects of the facility operations.
C. Plan, implement and evaluate a risk management program. Knowledge of risk management principles needed.
D. Plan, implement and promote integration between the facility and other community resources.
E. Plan, implement and evaluate an informational system which will help all employees to understand the organizational philosophy.

VII. Marketing and Public Relations
A. Plan, implement and evaluate programs that inform residents and community of the services offered.
B. Develop and monitor a process for communicating with residents, families, staff, volunteers, community, city and governing entities.
C. Develop referral communications with area hospitals.
D. Involvement in local community organizations or activities. Knowledge of community, social, educational and consumer organizations.

  • Ensures quality of resident care through planning, implementing, evaluating and delegating authority to appropriate staff to ensure that programs and care are maintained.
  • Prepares budget, and plans, implements and evaluates an integrated financial program with necessary financial controls in order to keep operations within budget.
  • Plans and implements the coordination of recruitment, hiring, training mentorship, retention, and evaluation of staff. Provides leadership in areas of employee and labor relations.
  • Directs and ensures compliance with all required government regulations, (e.g. Federal, State, Local, VA, OSHA, OFCCP, and various licensing agencies).
  • Plans future programs on growth, expansion, building and staffing by identifying objectives through strategic planning.
  • Plans public relations programs to enhance the image of all services in general.

Minimum Required Qualifications:

  • Bachelor’s degree in Healthcare Administration or related field; licensed as a Nursing Home Administrator in the state of MN or are able to obtain
  • Previous experience in a leadership position
  • Previous senior care experience
  • High degree of professionalism; ability to deal with extremely confidential information
  • Current valid driver’s license and proof of vehicle insurance
  • Ability to communicate effectively in both written and verbal formats, individually and to groups
  • Ability to use Microsoft Suite
  • Experience with budgeting and forecasting as well as strong networking experience
